Recently, we evaluated a horse for a suspected cardiac arrhythmia, and I wanted to take a moment to share insights into the diagnostic process, treatment considerations, and the overall impact of heart conditions in horses.


Diagnosing Cardiac Conditions in Horses

When assessing a horse’s cardiac health, we utilize a combination of diagnostic tests, including:

  • Electrocardiogram (ECG): Evaluates the electrical activity of the heart and detects arrhythmias.

  • Echocardiogram: A detailed ultrasound of the heart used to assess its structure and function.

  • Thoracic Auscultation: Listening to heart sounds with a stethoscope to identify irregular rhythms.

In this particular case, thoracic auscultation detected an abnormal cardiac rhythm, and an electrocardiogram confirmed atrial fibrillation. The echocardiogram revealed normal heart structure and function.


How Do Cardiac Arrhythmias Affect Horses?

A horse with an arrhythmia may experience:

-Decreased performance and stamina

-Poor recovery after exercise

-Failure to thrive

-Reduced life expectancy

While there is no breed predisposition, atrial fibrillation is more commonly seen in Thoroughbred racehorses under five years old.


What Are the Risks of Untreated Cardiac Arrhythmias?

If left untreated, cardiac arrhythmias can lead to:

Continued poor performance

Congestive heart failure


Warning Signs of Cardiac Issues in Horses

Owners should monitor their horses for these potential signs of heart problems:

Poor recovery after exercise

Decreased stamina during workouts

Failure to thrive


Treatment and Management of Atrial Fibrillation

For this case, we considered two treatment options:

Chemical Conversion – Using medication to restore a normal heart rhythm.

Electrical Conversion – A more advanced procedure that resets the heart’s rhythm using electrical stimulation.


If conversion back to a normal sinus rhythm is not possible, retirement may be the best course of action, as continued work can put the horse at significant risk.


Long-Term Monitoring for Horses with Arrhythmias

Routine monitoring is critical for horses diagnosed with a cardiac irregularity. This includes:


Regular auscultation (listening to the heart) by a veterinarian.


While there are no specific preventative measures, twice-yearly physical exams by a veterinarian can help detect issues early.
